drilling the through hole.

I am having a heck of a time getting a straight hole drilled in my wood blanks.

should I use a slow drill speed--fast speed?
I drill pilot holes, is that right or wrong.   

I am using a drill press an have to drill from booth ends .  getting the holes to meet is very hard

Help.
